National Health Service (NHS) is the name of the public health services in the United Kingdom. All applicants for Tier 1 Exceptional Talent Visa are required to pay for their future health services at Foggy Albion. This payment is called Immigration Health Surcharge (IHS). At the first glance, it seems that this step of the application is very easy to do. But in reality is not completely easy. This is my explanation why.
First of all, I have already mentioned in one of my posts that paying for IHS of the main applicant for Tier 1 Exceptional Talent Visa is not difficult. That is why, applying for 1 year Tier 1 Exceptional Talent Visa, you need to pay £200. Happiness will appear immediately :).
Reality is not the same. Dependants will pay the whole amount of IHS for 5 years.
Hence, if you are applying for 5-year visa, your Exceptional Talent Partner will pay £1000 for IHS and if you are the applicant for 1-year visa, the amount will stay the same – one thousand pounds.

Meanwhile, applying with dependents, the sum will differ. If your dependents will apply by themselves – the fee will be completely different.
By the way, if a visa application is refused or rejected the NHS Surcharge will be automatically refunded.
